Step by Step IVF Process With Time Line

Step 1: Starting The IVF Process – Diagnosing the Cause

During a regular menstrual cycle, hormones from the pituitary gland cooperate in the growth and release of an egg from a fluid-filled follicle within the ovary. While many follicles start growing each month, only one will be mature enough to ovulate. Ovulation is caused by an increase in LH about two weeks before menstruation starts

In the IVF process, Ovulation is triggered with a shot of LH or HMG (Human Menopausal Gonadotropin) and the eggs are harvested 36 hours later. The eggs are then fertilized with sperm in a petri dish and transferred back to your uterus where they can attach to your uterine lining.

The main objective of an IVF treatment plan is to stimulate healthy growth and release of eggs in the ovary. The mature eggs are then retrieved with a needle-thin telescopic surgical tool inserted through vagina and cervix under ultrasound guidance. The oocytes or egg cells obtained from the mother’s ovaries are then fertilized with the partner’s sperm in a laboratory.

Step 2: Egg Retrieval

The egg retrieval process is quite surgical. It takes a lot of medical attention. It can be said that the cost of most IVF treatments in India is spent on egg retrieval. Before you can schedule the egg retrieval process, your female partner will be given an injection called ovulation stimulation injection. This will make her eggs mature and she will also be put on an IV to improve the blood flow to the ovaries.

A vaginal ultrasound scan will then confirm that eggs have reached a mature stage and can be used for fertilization. Following this, sedatives are injected into the body of the patient before she is taken into surgery. The process may take 2-3 hours but following egg retrieval, the woman will be kept in an observation area for a few hours to make sure she doesn’t have any ill effects from the sedation.

After egg retrieval, it takes about 3 weeks for fertilization of the eggs and embryo development to happen.

Step 3: Sperm Retrieval

As soon as the eggs are collected, the fresh semen is derived from the male partner. If the male partner is not infertile, then this semen is naturally derived or else he has had surgery to extract sperm. After getting the semen, it is processed at a lab. The sperm is then washed and centrifuged to get rid of any debris or non-motile sperm. The washed sperm is then placed with the eggs in a petri dish and left overnight. Fertilization usually occurs within 12-24 hours after mixing the two together.

Step 4: Fertilization

In the laboratory, we use a microscope to look at sperm and eggs. The sperm and egg will stay together for two days while they become fertilized. If the fertilization happens successfully, then we will see signs that it is successful like blastocysts.

Step 5: Embryo Culture

A fertilized egg is put for 6 days to make it a blastocyst. The blastocyst has many cells. It is ready for the womb when it has more cells. Sometimes, the IVF doctors recommend freezing the embryo to increase the success of implantation. Research shows that a frozen embryo has higher chances than a fresh one.

Step 6: Embryo Transfer

The embryos will be graded by an embryologist and transferred into the uterus using a thin tube. You will be given some medication to help prepare your uterus for the embryos. The embryos are then placed in the uterus and left there to implant.

The whole process of egg retrieval, sperm retrieval, embryo development and embryo transfer usually takes about 2-3 weeks.
