What is GERD?
Gastro-esophageal reflux disease (GERD), gastric reflux disease, acid reflux disease, or reflux (in babies and young children) .
It is the disease of our stomach and esophagus (food pipe), where the acid (HCL) which gets secreted from stomach into the esophagus as a result of relaxation of the valves (situated at the lower end of our food pipe –they are like gated, which close after the food is passed into the stomach –preventing the contents of the stomach esp acid from entering into food-pipe. As the acid is corrosive in nature it can cause damage to the structure of food-pipe which can be temporary or permanent.
Changes can be like ulcer –breech in the lining of the stomach
Stricture –narrowing of the lower end of the gate of the food-pipe caused by acid entering the food pipe and causing damage –to the liming of the esophagus(food-pipe)—known as inflammation.
Prolonged entering of the acid in the esophagus –repeated damages to the lining –can lead to permanent change in the structure of the lining or alter its function –can be a potential factor for the development of cancer.
Causes of GERD
Obesity -In a large series of 2000 patients with symptomatic reflux disease, it has been shown that 13% of changes in esophageal acid exposure is attributable to changes in body mass index.
High levels of calcium –can increase the production of the acid –which can lead to increase acid production –giving rise to acidity.
A hiatus hernia may weaken the LES and increase the risk for gastroesophageal reflux. A hiatus hernia occurs when the upper part of the stomach moves up into the chest through a small opening in the diaphragm (diaphragmatic hiatus). The diaphragm is the muscle separating the abdomen from the chest. Recent studies show that the opening in the diaphragm helps the support lower end of the esophagus. Many people with a hiatus hernia will not have problems with heartburn or reflux. But having a hiatus hernia may allow stomach contents to reflux more easily into the esophagus.

Symptoms of GERD
The burning, pressure, or pain of heartburn can last as long as 2 hours and is often worse after eating.
Lying down or bending over can also result in heartburn. Many people obtain relief by standing upright or by taking an antacid that clears acid out of the esophagus.
Regurgitation— feeling of sour water coming from the food-pipe. Mouth getting filled with water. Giving rise to bitter taste in the mouth.
Less-common symptoms include pain with swallowing/sore throat, increased salivation (also known as water brash), nausea, chest pain, and coughing..

Diagnosis of GERD

But sometimes the symptoms may be prolonged and persistent –the inner part of the esophagus and stomach needs to be visualized.

The current gold standard for diagnosis of GERD is esophageal ph monitoring. It is the most objective test to diagnose the reflux disease and allows monitoring GERD patients in their response to medical or surgical treatment.
Endoscopy – is indicated Usually when the patients do not respond to the medical line of treatment and develop alarming symptoms like swallowing difficultly, blood in stools, anaemia, wheezing or persistent sore thorat or hoarseness of voice in long standing cases. In endoscopy –fiber optic tube is passed into the stomach and the condition of the mucus membrane (lining of the stomach and esophagus is visualized) for ulceration or structure change .
Biopsy- In cases where edema, swelling of the gland or the lining is observed a piece is extracted and sent for histopathological examination to rule out the possibility of cancer.
Treatment of GERD
Certain foods and lifestyle are considered to promote gastro esophageal reflux, but most dietary interventions have little supporting evidence. Weight loss and elevating the head of the bed are generally useful. Moderate exercise improves symptoms; however in those with GERD vigorous exercise may worsen them. Stopping smoking and not drinking alcohol do not appear to result in significant improvement in symptoms. Avoidance of specific foods and of eating before lying down should only be recommended to those in which they are associated with the symptoms. Foods that have been implicated include coffee, alcohol, chocolate, fatty foods, acidic foods, and spicy foods.
It is advisable not to lie down immediately after eating –as it will facilitate the passage of the acid upwards .It is advisable to walk for ½ an hr after meals and then lie down
The foot-end of the bed should raised while lying down in case of chronic regurgitation.
Treatment for GERD
Proton-pump inhibitors, such as omeprazole, are the most effective, followed by H2 receptor blockers, such as ranitidine. If a once daily PPI is only partially effective they may be used twice a day. They should be taken one half to one hour before a meal.
